Prodos 8 Crash

Hmm, a number of really annoying problems have been plaguing my GS lately...
 
Number one, whenever I start up GS/0S 6.0.1, it always stops and prompts me for
my Install disk... I remember there being some really dumb workaround for this,
but it's still annoying...
 
Secondly whenever I try to run anything ProDOS 8, I get some weird error
message, something to the extent of "sorry, sytem error #6219 has occurred" and
I have to restart. I can run everything by just pressing [8] at startup, but
it's still annoying to have that message.
 
And while I'm rambling, does anyone have a copy of a game called "Ancient Land
of Ys"?
