Thank you thank you thank you! That was the one I could vaguely
visualize (the sounds corresponding to the level elements being drawn)
but had no idea what the gameplay was like or what the game was
called. Just tried it via an emulator and... wow, great puzzles but
incredibly unforgiving.

I LOVED that game - independent control of left and right arms, flying
disks, teleporters, limb removal... just perfect. Well, an attempt, at
any rate. Definitely deserves a remake.

I don't believe so - I don't recall playing such a thing. The tune is
definitely familiar, but I'm not placing it now. :P
