If you have FTP access, System 5.0.4 is available on ftp.apple.com
(but not, as far as I know, on ftp.support.apple.com, Apple's main FTP
server).

I don't have the exact pathname, but it is in one of the directories
under the /dts/aii directory.

Probably not.  Simple tape recorders usually only have a microphone
input, which is very high impedance.  The IIgs has an output which is
suitable for an amplified speaker, which is reasonably low impedance.
You would need some kind of amplifier to equalise the signal levels.

A better solution is to install a stereo card with line-level outputs
into the IIgs, connect it to a stereo or tape deck with line-level
inputs, and record from that.

No.  You cannot have two video sources going to the same monitor.

It might be possible to connect the monitor to the game machine
_without_ the computer being connected, but I doubt it.  I think that
type of machine only provides RF-level output for a TV (or possibly
also composite output, which isn't supported by the IIgs RGB monitor).
