Where do I start for making a card?

I'm interested in designing a card for, I guess my Apple II+ (since it's 
the only one living right now) that could do I guess whatever I would 
make it do.  The question is what should I consult with to know where to 
begin?  I guess I'd like something that explains how the ports work, and 
possibly how the computer works in relation to them.  I know I'm starting 
rather cold turkey, but it's better then not starting at I all....

By the way, I remember seeing IIe's(?) and such in grey cases..when did 
Apple sell model II's in grey cases?
