LL fmtting Quantum80

  Just acquired a nice little 80 meg (3.5") Quantum ProDrive.
(80S) It doesn't want to format properly using my Apple //e and
an Apple scsi card. I think I may have termination problems.

Can anyone tell me how to set scsi id? Is this drive terminated?
Does the drive supply terminator power? or must it come from the
driver card?

I see two sets of jumper pins. one set labled A0, A1 AND A2.
The other set labled SS, EP and WS.  What are these?
I also note one socketed chip labeled TA201 DisCache.1988. Was
this an extra goody back then? Will the drive work without it?

Really appreciate any and all info about this drive.   DickP
