Apple II card ID

I'm trying to identify the following Apple II card:

It says Cardiocomp C1 Intelitool, Inc. Copyright 1986.  It
has only about 6 chips on it, a DIP, and a modular jack on
it, smaller than RJ11, but 4 pins -- similar to a phone
handset jack.  Chips are labeled XOR, CLK, BUF, AD, INA, and
VI A.

The largest chip (VI A) is Rockwell, R6522APE.

Any clues on what this is?
