I assume you are connecting it to a IIgs?  The pinout of the monitor
and computer are identical (the computer has a few signals that the
monitor doesn't use).

A straight-through cable can be used, connecting pins 1, 2, 3, 5, 6, 9
and 13.  You should use shielded cable, with the shield connected to
the outer metal of each connector.

The pin functions are:

 1  Red video ground
 2  Red composite video
 3  Composite sync
 5  Green composite video
 6  Green video ground
 9  Blue composite video
13  Blue video ground

The IIgs also uses pins 7 (-5V), 8 (+12V), 11 (sound) and 12
(monochrome video).
