Disk Initialization woes!

I have a problem. Earlier tonight, I formatted a 3.5" disk on my IIgs,
using the HFS format. Nothing went wrong. Later, when I attempted to do
the same with another disk, my system crashed. I did a OA-reset and tried
again. Again my system crashed. I shut down my hard disk and computer and
started them up again and tried to format. The system crashed again. I
reinstalled my system disk from my backup copy, and tried to format
again. (I of course reinstalled the HFS FST asl well.) It crashed again. The
thing is, it works when I format a disk with prodos. Can anyone tell me
what is going on?

I have a Rom 01 IIgs with 4meg memory, 7Mhz Zip GS, 80 meg HD, Apple High
Speed SCSI card and a Apple 3.5 drive. (Not UniDisk)

The point where it crashes is when I try to select the format I wish to
use. It crashes whether I select HFS via the mouse, or when I hit tab to
select the format box. However, if I just hit initialize without messing
with the format box, it will format correctly.

Oh, the same thing happens if I try to use the erase function to change
the format of the disk.

Please help!
