You might begin to suspect the hardware. I missed the beginning of this
discussion, so I don't know if you mentioned what type of SCSI interface you
have.
Check the cable to make sure it's securely fastened at both ends, and that
there are no kinks or tears in it.
Make sure you didn't change the SCSI ID of the drive, or the power-to-
termination settings.
Finally, it is possible that the drive and/or card are just dying.
