I recently (last week) had the same symptoms! I was adding a tape drive to my
SCSI chain. Here is what worked for me:
1. Originally, I had my HD's ID set at 5 and my new tape drive set at 3
I switched the order of these devices on the chain (HD now 3, TD now 5)
I don't know if this really had anything to do with the problem but I
did this along with the following so I can't say for sure.
2. I shortened the total length of the SCSI chain by getting a new and
much shorter cable to go between my HD and my TD. My computer-to-HD
cable was already only 2 ft. long. Try to keep your length down to
6 feet maximum.
After doing the above, my tape drive appeared in the SCSI device listing
and its device name (CIPHER ST150S 2/90) appeared intact in the listing.
(Prior to the cable change it would appear 'truncated' --PHER T150S 2/90)
