BIG time problems with my sytem please help.

Over the past several months i have been experiencing several boot and launch
problems that I am hoping someone could give me help with.
 
Sometimes when starting up (most of the time right now), the thermometer will
be going along but the inits with icons will start to really drag, by this I
mean that they will appear on the screen WAY later than they should. shortly
after the thermometer makes it to the end the system crashes.  If it doesn't
crash but starts to load the desktop one or two of my four partitions on the
HD will appear and then it will lock up.  If it loads fuly then it may still
crash when I launch proterm (it will happen with the occaisional other programs
but mostly proterm).
 
Most of (90% or better) these problems go away when I boot off of floppy, and
I can still use the HD.
 
I have totally stripped all non system inits, DAs, CDEVs, icons, and anything
else totally off the HD, and I still get these problems. I tried erasing the
volume and restoring everything from backup and still get the problems.  I
tried upgrading to sytem 6.0.1 (yes it took me awhile) and I still get the
problems (if not worse).
 
I tested my memory card (4 megs) and the test utility i have says all chips are
good.  It has been at least 3 years since I last installed any memory on this
board with no problems since then.
 
I have a Ramfast as my controller card.  I am not sure of the version.  I do
remember that 1.5 - 2 years ago I sent in my 15 (20?) bucks to get the upgrade
on the ROM.

I have a ZIPGS accelorator. don't remember the speed or memory on right now.

I recently had a symptom of some problem that I am not sure if it is related 
or not.  I recently went to start up my sytem and the HD wouldn't spin up, 
after smacking the case while trying to find the problem the HD started to 
spin up.  This happened twice in two days but not since, about 4 days now. 
 
last note, yesterday I went and optimized my HD and it has seemed to solve most
of the problems I have been haveing, except for the crashing while trying to 
launch proterm.  When it does crash I get an error theat look something 
like this:

With values in it of course.
