Problem with Vulcan 40 Drive

I'm having a problem with my AE Vulcan 40MB drive... (running on a ROM 1
GS)

Occasionally, when requesting disk I/O, the hard drive (all partitions) would
appear as if they had been ejected.  That is, they were greyed out and the
finder would ask me to insert it again.  I'm not sure what triggers the
problem.  A reboot will return things to normal (for a while).

I tried reformatting the drive, etc.  I thought it would fix, the problem but
apparently not!  Now, when trying to use the System 6 Installer, it sees the
hard disk as "ejected" and can't install.  What's the problem here?  Is the
drive physically damaged or something?  What utility can I get to fix the
problem?
