My guesses are:
- Integrated Woz Machine (IWM). I think this chip does most of the work when
  dealing with disk devices. It's located near the //c's back panel.
- Monitor ROM. Located under the keyboard.
- The motherboard itself. I hope it's not the motherboard that causes the
  trouble. It would be very difficult to detect and fix such an error.
