The Apple ///+ was the last ditch attempt (on Apple's part) to salvage
the Apple ///.  The original Apple /// was plagued by a heat problem -
if it got too hot inside, the socketed chips would pop out, causing
random crashes.  The ///+ sported a redesigned case, and a couple of
other enhancements.

It failed because of the bad press on the inital release of the ///.
