I've used twisted pair up to about 100 feet with no sign of problems
unless the wire is routed near a source of electrical noise (just like
a shorter cable).

Yes, using either a manufactured or homebuilt adapter.

The internal 16-pin game port (not available on the IIc or IIc+) has
support for two joysticks and three pushbuttons.

The pinout of the connector is available in the Reference Manual and
in several places online.  The wiring of a joystick is available
online in Sather's books _Understanding the Apple II_ and _Understanding
the Apple //e_.
