board does not have slot 0+4 because 16k and Z80 card are included.
The small card with the two EPROM chips goes into the "slot 0" position.
It contains regular Apple II+ firmware. IIRC the 2764 is $Dxxx/Exxx
and the 2732 holds $Fxxx with the Monitor. There is a 3rd /CS line so
you can fit three 2732 as well. The fourth socket is not used, only data
and address lines are connected.
As mentioned elsewhere, you should put the Z80's /RD pin back into its
socket and take the opportunity to check the other ICs as well. Maybe
somebody removed a chip during a repair attempt and put it into the
socket backwards.
