The IIgs's self test doesn't test anything on the memory card.  If you
have AppleWorks GS 1.1, try running MEMORYTEST.CL and see what it
thinks.  I don't think the OctoRAM came with a test program, but test
programs for most other memory cards should work.

I doubt that access speed is a problem, but it could be something to
do with the RAS/CAS access order of the RAM on the SIMMs (I can never
remeber which sort the IIgs requires).

I have had my OctoRAM working with 2 megabytes of 256k SIMMs (eight in
total), but never tried it with more than 4 megabytes using 1 MB SIMMs.

One possibility is DMA conflicts.  What SCSI card are you using?  The
OctoRAM is not DMA compatible in the second 4 megabyte bank.

You could try setting up a 4 megabyte RAM disk.  Check if the crashing
stops, and that the RAM disk retains its contents.  If so, it is
almost certainly DMA trouble.
