Mmm...  in addition to the potential problems with a really fast accelerator,
I find it's a good idea to hit Ctrl-Reset on GSs after entering PT3...
stuff about clearing the interrupt buffer, and all that...  :)

If that doesn't help, it's possible that it's a problem in your TransWarp, or
some other piece of your system hardware -- as inconvenient as I know it is,
you might try pulling various system accessories, and see if that helps... 
Personally, I'd start with the TransWarp since accelerators tend, more than
other stuff, to be the cause of flakiness, in my opinion...  it at least gives
a good starting place...  :)
