I can check the CP/AM source code to remind myself what was used there. I do remember that it's the same scheme as Wink Saville used to support 3.5" disks under the Applicard system. However, Cirtech and ALS (I _think_) also supported 3.5" CP/M disks and there's little reason to expect they used the same layout.
