You have one of the very first cards. I have later found and fixed a
bug that prevented writing.
As I recall I have mentioned this to you but never followed up. Sorry.

You can upgrade to either my fixed firmware (on my page) or the
vladitx's version.
It is likely much better for ProDOS - "don't buy software from a
hardware company".
Or use my fixed firmware if you want to also use nibble images.
