I found it very easy to set up the control on my server.  I have included the 
HTML that I use (the arguments are modified slightly to protect the actual 
content):

It looks like I wrote that five years ago.  Just make sure your files and 
directories have the correct permissions.  That was the only gotcha that I ran 
into.
