Well, you wouldn't, really. If you're talking about Deja Vu II from
here:

then it needs to run under GSOS. That web page has a hard drive image
you can run under most GS emulators that will boot GSOS and also the
game.
Incidentally, to format a 3.5" disk to boot, you need a ProDOS
utilities disk. You use the utilities to format it, then copy PRODOS
and BASIC.SYSTEM to it - then it becomes bootable. But there's no
built-in "INIT HELLO" sort of thing in ProDOS land.
