I cannot answer that question but I just read the following in the
Apple's "The Personal Computer Magazine and Catalog" ((C) 1979, Volume
1, Number 2 on page C-12):

"Programmers Aid #1 is packaged in a single 2K-byte ROM to be inserted
in a socket of the APPLE II. The routines upon which it is based are
completely documented in the manual which accompanies the package.
(Order No. A2M0019. Note: this ROM is now included in APPLE II
computers.)"

So it started out as an extra cost option but then became a no-cost
standard feature (of APPLE II systems).
