Just to ask the obvious question, do you have HS.FST (the ISO 9660 and High 
Sierra FST) installed in the system/fsts folder?

The Apple II FAQ also reminded me that in some cases you might need the data 
CD to be in the drive when you boot.
It said "Some drive + card combinations may not go on-line if no CD is 
present by the time you turn ON the computer."
