Plug in the Apple, turn it on.  You hear a beep.  Hold down the Ctrl
key, hit the Reset key.  You'll see the Applesoft prompt ("]").  You
can now do BASIC stuff; and you're right, you don't get to save
anything on disk (though you could on tape, if you hooked a tape
recorder up).

Not reasonably possible.  You'd need new mechanisms, new connectors,
and a new controller.

You can create your own if you have a serial card... see below.

Not unless it's external, and you have a serial card in your Apple.  An
internal modem won't work.
