I've had very good results with just 150 dpi, when using "oversampling"
during the actual scan. The newer Epson scanner software at least can do
this very easily - it's got some stupid, easy to trademark but hard to
remember name though, I think it was "unsharp mask" or some such. I
suspect other manufacturers offer this option too, under different (but
probably still stupid) names. The idea is to scan more pixels and
average them out, just like Font Smoothing and such things do for output.

I always scan as TIFF, then convert to PNG, and finally use PDFCreator
for the output, with its default settings (except maybe for paper size
and orientation). The PNG step is required since somehow PDFCreator
and/or the Windows Imaging stuff isn't good at going from TIFF directly
to PDF. As PNG is lossless, only time is lost in the intermediate step,
not quality.
