Control-Reset is unnecessary, and the Open Apple key doesn't exist.
from BASIC prompt:
PR# (slot number)
This works from both Integer BASIC and Applesoft. IN#(slot number) works
as well.
from the monitor:
Cx00g (x = slot number)
Other possibilities: 6[CTRL-P][RETURN] or 6[CTRL-K][RETURN]. Provided
that 6 is your slot number.
to enter monitor from BASIC:
call -151
Of course on this early model the opposite is more needed, because it
will start in monitor (asterisk prompt) mode.
To enter BASIC from the Monitor, initializing BASIC on the way (you need
to do this when first entering BASIC):
[CTRL-B][RETURN]
Without initializing Basic (program is not destroyed; won't usually work
when first entering BASIC):
[CTRL-C][RETURN]
Both of the above unhook DOS. After DOS is loaded, replace them with the
three-dogs command:
3D0G[RETURN]. That's a numeral zero, not letter Oh.
