HP ScanJet 3p flatbed scanner (SCSI, monochrome/greyscale).
I scan at 300 DPI black and white (aka monochrome or bi-level).
Adobe Acrobat 5 to convert the TIFF scans to PDF.  I keep the
TIFF files in case I want to do OCR.  I unbind the books
before I scan them.  If any scan looks skewed, you may assume
the page was straight on the scanner and the original page was
printed crooked.
