Applesoft was never published by Apple, but there were numerous
commented disassemblies made available, notably the one on the
reverse side of the Merlin assembler disk (for use with its companion
disassembler.  The reason for its lack of official publication was
no doubt the fact that it was copyrighted by Microsoft as well as
Apple.

Integer BASIC is a special case, since no assembler source was
used in its construction, except in a manuscript by Woz.  It seems
likely that someone would have created (disassembled) a source for
it, but I have seen no information regarding the original manuscript.

I seem to recall that the self-test firmware in the IIe and IIc wasn't
listed.

At least the self-test firmware for the unenhanced IIe is listed,
and I think I recall the enhanced version, as well. (?)

The IIc listing for the "UniDisk 3.5" ROM includes the bank-switched
portion of the ROM, including the rudimentary AppleTalk code, SmartPort,
etc.
