What model mac are you running? Does it have an internal floppy? I don't a
usb floppy will write to DD disks. Which also brings up the question, are
you using double density disks?

Disk copy and the newer manual-inject drives used on later "old-world rom"
macs (which if you are running OS 9.2.2 I'm sure you have) are usually
unable to write bootable Apple II disks. The best bet is to try and find an
old mac with an auto-inject drive (no black drive door). If this is
impossible, I've heard people have had luck using a program called Disk Dup+
instead of Disk Copy to write Apple II floppies.
