Open it with CiderPress and hit Ctrl-I (or File->Archive Info).  That'll
tell you what format it's in.

If it's an unadorned DOS-order 5.25" disk image, you can transfer it
with ADT and a serial cable.  If not, you can use CiderPress to convert
it first.

Another approach is to convert it to a ShrinkIt .SDK file and then use
a ZMODEM transfer through a serial cable, and unpack it with ShrinkIt on
the Apple II.

There's no way to write an Apple II 5.25" floppy from a PC.
