The easiest way to format and copy files is booting 5.25" floppy disk
version of "Copy II Plus v7.2 or newer". 3.5" disk drive is a device
listed as 'Slot 5  Drive 1' and can copy files between 5.25" and 3.5"
floppy disk drives.

Formats for ProDOS (not DOS 3.3), but isn't bootable until copying two
files from bootable ProDOS disk [not copier program]: PRODOS.SYS and
BASIC.SYSTEM.

Without a copier program, format the disk by booting 5.25" ProDOS disk,
which automatically loads into ProDOS Basic and type: Catalog,D1,S5 and
Init Hello.
