The last time I needed to scan a document into a PDF, I did something like
this:
1) Create multiple high-resolution monochrome TIFFs with either the software
that came with the scanner or the software that's built into Windows.
2) Use tiffcp (part of libtiff, http://www.libtiff.org/) to combine the
multiple TIFF files into one TIFF.
3) Use tiff2ps (also part of libtiff) to convert the TIFF to PostScript.
4) Use Ghostscript (http://www.ghostscript.com/) to convert from PostScript
to PDF.
(I suppose you could also use Acrobat Distiller for the last step, but
Ghostscript is free (as is libtiff) and often does a better job anyway.)
Ghostscript and libtiff are both available as part of Cygwin. There might
also be native Win32 builds of each, but I've not gone there. I suspect
that libtiff and Ghostscript also run on Mac OS X; they might even ship with
it.
