     First, are you sure the diskettes you are trying to format are DD (double
density) type diskettes? Most Apple II 5.25" cannot format the 1.2MB 5.25"
diskettes (which are usually what you find in computer stores).

     Have you tried formatting a diskette under DOS 3.3 (instead of Copy II
Plus)?-- i.e. boot DOS 3.3 and get to the BASIC prompt and stick in a fresh
diskette and enter INIT HELLO.

     If you are using DD diskettes, what stuff can you do? Like, can you boot
diskettes okay? Can you write files to diskettes?
