In fact my approach answers both questions :-)

I start out with an emulator disk image ( a *.dsk file) with DOS 3.3
(_NOT_ ProDOS) that has enough room for the binary file. Any DOS 3.3
"system disk" / "system master" you find online should be fine.

Then I use the "a2tools - utilities for transferring data between Unix
and Apple II DOS 3.3 disk images." from Terry Kyriacopoulos to insert
the binary into the image:
a2 in b <imagefile> <a2file> <pcfile>

Then I use ADT (Apple Disk Transfer) to transfer the emulator image
via a serial link to a DOS 3.3 floppy disk.

Then I use the ProDOS System Utilities to copy the binary file from
the DOS 3.3 disk to a ProDOS disk.

This should get you a BIN file with a base address of $2000. If you
need a SYS file use some conversion tool or just BLOAD the file and
BSAVE it with TSYS.

The last two steps could optionally be performed in an A2 emulator
before transfering the resulting ProDOS emulator image to a ProDOS
floppy disk.
