Glen Bredon's DOS Master, perhaps? I downloaded that one a while back
and had no luck at all with it... I seem to recall not being able to
successfully format anything for DOS and afterwards having a helluva
time reformatting as prodos. Naturally, operator error is the most
plausible explanation ;-)
