Yes, that is the Apple 5.25" drive controller,
as it plainly states on the card.

The card for use with the UniDisk 3.5" drive
is called "Apple 3.5 Floppy Disk Drive Interface
Card". Mine says Apple II UniDisk 3.5 on the box.
