The game usually seems to get listed as "Artillery", since that is the
name of the Applesoft BASIC file. (Our copy of the game is listed that way,
too; but, when run, it shows as "Super Artillery".)

You should be able to find it on the Horde, which is an Asimov mirror.
(Asimov seems to be down right now.)

     Go to ftp://ftp.au.horde.org/pub/apple_II/images/games/file_based/ .

The disk image file has a long name (listing the games on the disk)
beginning with "artillery_ballistics_divebomber_".
