I have found a card labeled Microtek RV611, the output cable is like a
5.25" PC drive.

Anyone seen one of this or know what it is?

It's a parallel port card with the wrong cable on it. The cable should have
a IDS34 on one end, and a Centronics 36 Male on the other end.
Back about mid 80's I bought about 20 of them cards brand new and sold them
at a good profit.
