They got taken out for the ][+.  They were present in the original
monitor (non-autostart ROM, as used in the original Apple ][, now known
as the "old monitor ROM") but there wasn't enough space to retain them
in the autostart ROM (][+).  The IIe doesn't have them either.

They are available in the ROM 3 IIgs, but I don't recall whether any
revision of the IIc has them.

IIgs ROM 00 and ROM 01 have hooks to add RAM-based step and trace
handlers, but I don't recall anything making use of this feature.
