Just a follow up in case anyone else ever needs this info.

Here is the dip settings I found on the net
(http://user.chollian.net/~spix/dip_switch.html)  I can't vouch for
the accuracy of all of them, but I can vouch for the 300 and 19200
baud settings for the serial port which is what I used.

Also, I could not get this card to work with ADT proper, but I was
able to get it to work with ADTcc just fine.
