MFM and GCR are two different low-level formats. CP/M is an operating
system and a particular high-level file system format.
To a rough approximation, MFM is to GCR as CP/M is to Apple DOS (or ProDOS).
When you use CP/M on an Apple II (using a standard Apple disk controller),
the disk format is still GCR.
