Appendix D of Beneath Apple ProDOS has a great deal of detail. Of possible
interest is the footnote at the bottom of page D-9:
"** See table D.1. Notation used here is borrowed from _Understanding the
Apple II_ by Jim Sather."
Sounds like Sather's book(s) are the definitive work.
