The Sup'R'Mod with which I'm familiar has a 4-pin connector that plugs into
the RF-modulator jack on a II/II+/IIe motherboard.  The photos you provided
show something that looks very much like the RF modulator that came with my
TI-99/4A (main difference that I can see is that the TI modulator had one
cable carrying video, audio, and power to it with a DIN-5 plug on the end). 
I made an adaptor years ago that allowed me to plug the TI's RF modulator
into my IIe...while it pulled the video signal from the RF-modulator jack, I
don't recall offhand what voltage the modulator needs.  You could try asking
in comp.sys.ti...for that matter, I've gone ahead and cross-posted to that
group.  Once you have the correct voltage, figuring out the polarity should
be trivial...the lid snaps right off the modulator case (it might need to be
"persuaded" with a screwdriver).
