I think your problem here is OS Format. What's going on is that CP/M
formats it's disks differently than ProDOS. From what I recall,
FishWings only supports ProDOS disks. So when you create a disk image
and drag files to it (even CP/M files) you still end up with a ProDOS
disk. You should use an emulator that can run Apple II CP/M, using
the built in CP/M file copy tools to get your files onto your disk
image.
If you have a Macintosh there's a fantastic (IMHO) utility that can
create/read/write CP/M disk images directly, called ADFS (Adfiss?) no
emulator necessary.
