For your information: this ROM version is normally known as the
"revision C firmware" for the original Apple SCSI card.

If you have access to an EPROM programmer and a 27C128 EPROM, I can send
you a copy of the firmware. (I have it as a simple binary image, which
I can send as an E-Mail attachment.)

If you attempt to boot a IIgs into GS/OS, it checks the firwmare version
on the original SCSI card and will refuse to use it if the firmware is
too old (i.e. not revision C). This is because GS/OS is not fully
compatible with the older firmware, and there are bugs in the older
firmware which limits its reliability.
You would be able to boot into ProDOS-16 or ProDOS-8 and use the SCSI
card to access a SCSI hard drive (assuming you have everything else set
up correctly).
