As Rubywand recently mentioned Asimov2 can create/extract 2mg images.
It can be found at:
http://www.apple2.org.za/gswv/a2zine/Utils/
It runs on an Apple IIgs. At the present I believe only the Apple IIgs
emulators support the 2mg images.
