My uncle built this case, I do know that.  He either had the board and ROM 
board fabricated from plans, or it came in a kit form, with no case.

My uncle used a standard Apple II/+/e power supply when he made it, and it 
has the standard Apple connector.

Mine also has green slots.

I'm not interested really in making a working machine.  Actually, I am 
attempting to appraise it's value for sale to a collector, along with all 
my other Apple II+ stuff.  My IIc and KEGS pretty much satisfy my Apple 
cravings, except for CP/M, and I am looking for a Z-RAM for that.

It says "Apple ][", and my uncle also burned the EPROMs himself.  Matter of 
fact, I own the II plus who's ROMs were copied for it.
