Thanks for the reply...

Would you be willing to:

- Take pictures of your cards so I can see chip names, component IDs,
etc? I realize there are some PALs, but reading and programming PALs
is not an insurmountable task.  I don't have the equipment to do it,
but I'm sure I can find someone who does :-)

- Make a HEX dump of the EPROM and email it to me?

If you cannot do the above, then since you have four of them :-),
would you be willing to send one to me for analysis?  I have the
ability to get the EPROM contents, and physically having the board
will allow me to make a parts list.  I would be willing to buy the
card from you, or send cash as a deposit, to be returned when I return
the CMS card to you.....

Either way (sending me a card, or sending pictures/EPROM), you've
secured a blank for free (I'll even pay shipping)... Since I've gotten
about two other private replies so far, you can even have two blanks!
