You are asking me to trawl back in my memory banks :)
There was a DOS3.3 one released by a Canadian Apple User Group and distibuted
by the International Apple Core. It crashed if you tried to enter anything in
the DOS area (9600?).
Nibble did a couple in Dos3.3 and ProDOS.
Remove the x's from my email address to contact me. Let me know if you want 
.shk or .dsk format files.
